We collect, use and manage personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs), as well as our professional and clinical obligations under AHPRA standards and NDIS Quality & Safeguards Commission requirements.

We may collect personal and sensitive information necessary to provide safe and effective nursing services. This includes personal information such as your full name, date of birth, contact details, emergency contact information, NDIS number and plan details, and guardian or nominee information.
We also collect health and clinical information, which is considered sensitive information under privacy legislation. This may include medical history, medication information, continence and bowel or bladder information, wound assessments, allied health reports, hospital discharge summaries, risk assessments and individualised care plans.
In addition, we may collect information related to incident reports, feedback or complaints, service agreements, billing details and funding information. We only collect information that is reasonably necessary to provide our services or meet our legal and regulatory obligations.

We may collect personal information directly from participants, families, guardians or nominees. Information may also be obtained from support coordinators, allied health professionals, hospitals, healthcare providers, referral documentation or through website contact forms.
Where possible, we collect information directly from you and with your consent.
Why We Collect Your Information
We collect and use personal information to provide specialist disability nursing services and to develop, implement and review individualised care plans. Information is used to manage clinical risks, coordinate care with other providers, prepare NDIS reports and funding documentation, comply with legal and regulatory obligations, process service agreements and payments, and improve service quality and safety.

We will not use or disclose your personal information for purposes unrelated to your care without your consent, unless required or authorised by law.
Disclosure of Information
We may share relevant information with support coordinators, allied health professionals, SIL providers, hospitals, medical practitioners, plan managers, and government or regulatory authorities where required.
Information is disclosed only on a need-to-know basis to support safe, coordinated and effective care.
Data Security
We take reasonable steps to protect personal and sensitive information from misuse, interference, loss, unauthorised access, modification or disclosure. This includes the use of secure electronic record systems, password protection, access controls, secure storage of physical documentation, confidentiality agreements for staff, and regular review of data security practices.
When information is no longer required, records are securely destroyed or de-identified in accordance with applicable legal retention requirements.
